Contact the major credit bureaus

Ask for a fraud alert to be placed on your file by all three credit bureaus. The alert tells lenders and other users of credit reports to be careful before opening or changing accounts in your name. Also consider a follow-up letter. The special toll-free numbers for the fraud departments are:

Contact other creditors

Contact other creditors, including credit card and phone companies, banks, and other lenders, to notify them of potential fraud or identity theft. Consider following up your telephone conversations with a letter. Each creditor may have a different process for handling a fraud claim. Make sure you understand exactly what is expected from you, and then ask what you can expect from the creditor. At the conclusion of an investigation, ask the creditor for a document that states you are not responsible for any unauthorized transactions.

Close any accounts that have been breached and reopen with new account numbers and different passwords. Do not use your Social Security Number as a username or password.

File a report with the local police

Fill out a police report that will detail what happened and get a copy for future reference.

Report the criminal activity to the Federal Trade Commission (FTC)

Call toll-free 877-IDTHEFT ((877) 438-4338). Fill out a complaint form online at the FTC’s ID Theft website at https://consumer.ftc.gov/identity-theft-and-online-security/identity-theft. Or, mail a letter to: Identity Theft Clearinghouse, Federal Trade Commission, 600 Pennsylvania Avenue, NW, Washington, DC 20580. An "ID Theft Affidavit" available on the FTC website also can be used to help you prove you are an innocent victim and help you keep debts you did not incur from appearing on your credit report.
